Dr. Connie Shim Middleton grew up on the southwest side of Fort Wayne and attended Homestead High School. Dr. Shim continued her education at DePauw University and received her doctorate of dental surgery at Indiana School of Dentistry in 2012. During her time at dental school, she was awarded the Isaac Knapp Dental Education Foundation Scholarship and was on the Indiana University School of Dentistry Dean’s List. She currently serves on the Young Dentist Committee and is a board member on the Issac Knappe District Dental Society Committee. Dr. Shim enjoys working with children and adults. During her free time, she plays tennis and golf and likes spending time with her family.
Dr. Logan Goodrich grew up in Huntertown, Indiana and graduated from Carroll High School in 2011. At Wabash College, he earned his Bachelor's degree in Biology and then went on to earn his Masters in Biology from IUPUI. Dr. Goodrich moved east and graduated among the top 10 in his class at Tufts University School of Dental Medicine in Boston. Dr. Goodrich is excited to be back home in the Midwest and to provide dental care to his community. He is an active volunteer with St. Martin Healthcare in Garrett and in his free time, he enjoys spending time with family, hiking, fishing, and playing with his Jack Russell, Zoe. Dr. Goodrich is a member of the Academy of General Dentistry, American Dental Association, and Isaac Knapp Dental Society.

Dr. Brendan O'Hara grew up in Fort Wayne and graduated from Homestead High School in 2006. He attended Indiana University and earned a degree in Kinesiology before going on to get his Masters in Biology at IUPUI. Dr. O'Hara went to dental school at Tufts University School of Dental Medicine in Boston, MA and returned to Fort Wayne with his wife, Kelly, to practice and raise their family. Dr. O'Hara loves performing all aspects of dentistry including root canal therapy, implants and cosmetic restorations. In his spare time, he enjoys playing golf, running, spending time with his family and volunteering at Matthew 25. Dr. O'Hara is a member of the AGD, ADA, IDA and Isaac Knapp Dental Society and a member of the Young Dentist Committee in Fort Wayne.
Mat grew up 25 minutes south of Fort Wayne and attended Norwell High School. When deciding on a location to practice dentistry, it was important for Mat to be near the family and friends he grew up with as well as to serve and give back to the community that helped raise him. Mat graduated near the top of his class and is educated in the newest techniques in cosmetic, implant, and restorative dentistry. Dr. Mat is co-owner and has been practicing since 2010 after graduating from Indiana University School of Dentistry. He is comfortable with performing root canal therapy, implants, and cosmetic braces. In his free time, he enjoys traveling, running, and golfing.
